Instructions to Sponsors:
Please provide complete and meaningful information to assist the BCA Board of Governors in determining the Applicant’s eligibility under the provisions of the Constitution, By-Laws and Code of Ethics. Information provided will be kept confidential. It is required that the Sponsor knows the Applicant and can attest to the Applicant’s experience and compliance with the above. Sponsors may be contacted to provide additional information regarding the Applicant. Sponsors must be members in good standing in BCA.
The information requested below will be used for the sole purpose of providing appropriate information for consideration of the Applicant’s membership by the BCA Division Board of Governors.
